Disclaimer & Waiver Form

I understand that I am solely responsible for any harm whatsoever caused by my dog(s) while my dog(s) is/are attending daycare at Doggy Haven Guesthouse & Daycare. I also understand and agree that in admitting my dog(s), Doggy Haven Guesthouse & Daycare has relied upon my representation that my dog(s) is/are in good health and have not harmed or shown aggression or threatening behaviour towards any person or any other dog.

I further understand that due to the ways dogs interact with one another, minor cuts, scratches and injuries can occur even though the dogs are carefully supervised. While my dog(s) is/are in the care and custody of Doggy Haven Guesthouse & Daycare, if I am unreachable in the event of an emergency, I hereby authorise Doggy Haven Guesthouse & Daycare, to seek immediate veterinary care for my dog(s). I understand that all costs in connection with, veterinary, medical or other treatment shall be my responsibility and for my own account.

I hereby release and agree to save and hold harmless, Doggy Haven Guesthouse & Daycare, it's employees and assistants from any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain, or which may be caused in any way by my dog(s). I specifically, without limitation, agree to fully indemnify Doggy Haven Guesthouse & Daycare for all such liability, claims, suits, actions, losses, injury or damage.

I understand that there are risks and benefits associated with group socialisation of dogs. I accept the risk and desire a socialised environment for my dog whilst attending daycare or boarding at Doggy Haven Guesthouse & Daycare.

Although every care will be taken for the dog/s, I understand and accept that it is still possible that during normal play, dog/s may receive minor nicks and scratches, injuries and/or any transmissible virus from playing with other dogs.

I certify that I have read and understand the rules and regulations set forth herein and that I have read and understand this agreement. I agree to abide by the rules and regulations and accept all the terms, conditions and statements of this agreement and confirm the truthfulness of the contents of the information form completed by me.

Although we carefully screen all applicants, occasionally we discover that this is not an appropriate environment for every dog. Doggy Haven Guesthouse & Daycare reserves the right to permanently remove any dog from daycare or boarding at any time. We are an exclusive daycare for small breed dogs and puppies over 6 months of age and reserve the right to disallow any unsuitable dogs to participate in the daycare or boarding at any time. No fees will be refunded in this case.

Our daycare and boarding facility are limited to 20 dogs per day to allow for personalised hands-on attention for each dog. This is the cornerstone of Doggy Haven Guesthouse & Daycare – we only want the best experience for the dogs here, whether it be at daycare or boarding.

No dog/s will be released to any unauthorised persons unless prior written consent with adequate identification is received. The safety of your dogs is our first priority.

Unless Doggy Haven Guesthouse & Daycare cancels daycare for the day due to extreme/dangerous weather or an emergency, you will be charged for the day unless your pet is ill, and 24 hours' notice thereof is given prior. In cases of sudden onset of illness or veterinary care required, Doggy Haven Guesthouse and Daycare will assess on a case-by-case basis.

Please note that we run Doggy Haven Guesthouse & Daycare solely, your dogs are our passion and not a business endeavour. Our commitment is to care and love for all dogs at Doggy Haven Guesthouse & Daycare. This means that we work strictly by appointment as this allows us to do our job to the best of our ability and to give the dogs the attention and dedicated care, we pride ourselves on. Times for drop off and collection are to be prearranged with us and adhered to. Extenuating circumstances, traffic delays and other unforeseen occurrences need to be communicated timeously. Your assistance in this regard is paramount to being able to provide the best service that your dog/s deserve. We do not have strict drop off and collection hours to assist with your schedule and for your convenience, however the pre-agreed times for collection and drop off always require your compliance to provide the best service to you and your dog/s.
